Interviews with the Unit Manager and the Administrator revealed that the Smoking Assessment was inaccurate, as the resident's cognitive impairment should have precluded independent smoking. These deficiencies highlight the facility's failure to accurately assess and document residents' behaviors and cognitive abilities, which are critical for ensuring appropriate care and safety measures. The inaccuracies in the assessments for both residents were confirmed through staff interviews and record reviews, indicating a lapse in the facility's assessment processes.
