The Director of Nursing (DON) and other staff members acknowledged that the care plan interventions were not followed as required, and there was a failure to ensure the pod doors were closed and residents were monitored as per the care plan. This failure to implement the care plan interventions led to the resident's elopement, posing a risk of serious harm.
