The facility failed to implement its written policies and procedures to prevent abuse, neglect, and theft, affecting all four residents reviewed in the sample. The Director of Behavioral Services incorrectly believed that no incident report was needed if no physical injury occurred. The facility's policies require incident reports for all resident-to-resident altercations, regardless of injury, and mandate that abuse assessments and care plans be updated after each incident or allegation.
