However, the DON was not aware that the resident's contractures were not care planned and believed there was no risk to residents if care plans were not up-to-date due to other communication methods among nursing staff. The facility's policy on comprehensive care plans emphasized the importance of ongoing assessments and updates to care plans when there is a significant change in a resident's condition.
