Summary

The facility failed to establish and maintain an effective infection prevention and control program, as evidenced by multiple instances of staff not adhering to hand hygiene protocols. Observations revealed that several Licensed Vocational Nurses (LVNs) and Certified Nursing Assistants (CNAs) did not wash their hands or use hand sanitizer before and after providing care to residents. This included medication preparation and administration, as well as providing incontinent care.
