The Administrator also stated the PASARR was not accurate and another level one needed to be conducted, but none could be located in the EHR.

The Administrator stated the Social Worker was responsible for submitting Level II PASRR requests and acknowledged that, based on this prior guidance, a Level II request for this resident was not completed.
