The HR representative confirmed that there was an issue with the computer-based training system, which failed to trigger the required trainings for staff. The facility's Training Programs policy, dated October 2021, stated that employees would be provided with the necessary training to meet state and federal regulations, and that the Director of Staff Development and HR Director were responsible for ensuring these training needs were met. However, this policy was not effectively implemented, leading to the identified deficiencies.
