Summary

The facility failed to provide a sufficient number of staff to ensure residents received the needed care and services. Resident #2, who had diagnoses including psoriasis, skin changes, and moderate protein calorie malnutrition, required assistance with bathing. Despite being scheduled for showers on specific days, the resident received only four showers out of 13 opportunities in March 2024, with multiple refusals and missing documentation. In April 2024, there was no documentation of the resident receiving a shower on their scheduled days. The resident reported not receiving a requested shower on April 7th, and the shower sheets lacked proper documentation for several days. Resident #4, diagnosed with hemiplegia, hemiparesis, muscle weakness, and lack of coordination, was dependent on assistance for bathing. Elopement of Wandering Resident and Delayed Call Light Responses
