Survey Results Not Readily Accessible

Boro Park Center For Rehabilitation And HealthcareBrooklyn, New York Survey Completed on 05-06-2024

Summary

The facility did not ensure that the survey results were posted in a place readily accessible to residents, visitors, or legal representatives. The survey results were located inside a binder placed behind the Security desk, and there were only two notices posted about the availability of the survey results, which were not in conspicuous locations. During multiple observations, survey results could not be located throughout the facility. Residents at a Resident Council meeting stated they did not know where to locate the survey results, and it was observed that the notices were not prominently displayed. The survey results were not visible at the security desk, and residents or family members would have to ask the security officers for the survey results, which were kept in a black binder without a label on the spine indicating its contents. The Administrator stated that the survey results signage was located in the lobby after the elevators and considered this a conspicuous place. However, the Administrator was not aware that signage should be posted in more than one location and that survey results should be available without being asked for. The facility policy and procedure titled Survey Results, revised in October 2015, stated that copies of all survey reports along with approved plans of correction are on file in the administrative office and must be readily accessible for viewing. The policy also documented that residents, visitors, etc., should not be required to ask to see the results.

Short Summary

Survey results were not readily visible for residents and visitors to access without asking. The admin stated the survey book was kept behind the front desk in a holder, but a chest-high partition blocked it from view even though a posted sign said the annual state survey results were available and readily accessible 24 hours daily. During a resident council interview, a resident said the survey results were there, but they had to ask for them.

Residents Unaware of Access to Survey Results Binder
E
F0577 F577: Allow residents to easily view the nursing home's survey results and communicate with advocate agencies.
Short Summary

Facility staff did not ensure that residents knew they could review the survey results binder or where it was located. In a resident group meeting with the council president and several residents, all attendees reported they were unaware of their ability to access the survey book and could not identify its location, with one suggesting it might be behind the nurse’s station. The Activities Director stated that residents were educated at each resident council meeting about the binder’s location and that this was documented in council minutes, but no approach was described for updating residents going forward. When these findings were presented to the Interim Administrator, DON, ADON, and a corporate nurse consultant, they offered no comments or concerns.
