The MDS Coordinator and Social Services Designee were responsible for creating and revising the care plans, but the care plans did not accurately reflect the residents' conditions. The Assistant Director of Nursing acknowledged the need for appropriate interventions and activities for dementia residents, but these were not implemented in the care plans reviewed.
