Resident #14 was transferred to the hospital and returned over a month later, Resident #24 was transferred and returned within a week, and Resident #45 was transferred and remained hospitalized at the time of the report. None of their medical records contained documentation that the bed hold policy was communicated at the time of discharge. Interviews with facility staff revealed a lack of awareness and clarity regarding the responsibility for completing bed hold notifications. The administrator indicated that the charge nurse is responsible for completing bed holds but believed that the licensed nursing staff were not aware of this requirement. An LPN confirmed that bed holds should be given to residents sent to the hospital but could not explain why they were not completed. The DON, who was new to the position, acknowledged knowing about the form but was unsure who was responsible for completing it.
