Summary

The facility failed to develop, implement, and maintain an effective nurse aide (NA) in-service training program as determined by performance reviews and the facility assessment. The facility did not provide a policy regarding NA training or performance reviews, nor did it provide any training records or performance reviews. The facility assessment, dated 02/12/23, was not reviewed annually and contained inaccuracies regarding the number of residents with intellectual or developmental disabilities and behavioral healthcare needs. The ADON is currently handling many of the DON's responsibilities due to the absence of a current DON. The Administrator confirmed that performance reviews for nurse aides had not yet been initiated.
