Staff members, including Certified Nursing Assistants and Licensed Practical Nurses, were observed providing care without the required PPE. Interviews with staff indicated a lack of awareness or implementation of EBP, further highlighting the facility's failure to adhere to infection control protocols. This deficiency increased the potential for the spread of infection, cross-contamination, and disease transmission among residents in the facility.
