Failure to Prevent Resident Elopement

Summary

The facility failed to complete an elopement risk assessment, develop and implement an elopement care plan, and provide a secure physical environment for a resident with a known history of successful elopement. This resulted in the resident eloping from the facility without staff awareness. The resident was found two days later at his mother's home. The resident had a history of Major Depressive Disorder, Paranoid Schizophrenia, Generalized Anxiety Disorder, Obsessive-Compulsive Disorder, Delusional Disorders, Bipolar Disorder, and Hallucinations, and was cognitively intact with a BIMS score of 15.
