The facility failed to develop, implement, evaluate, and reevaluate a plan to prevent continued unplanned weight loss for a resident (R14). However, these interventions were not consistently implemented or documented, leading to continued weight loss. The facility's policy on care plans emphasized the need for comprehensive assessments and individualized care plans, but this was not effectively executed for R14, resulting in a failure to prevent further weight loss.
