Additionally, Residents 15, 18, 31, and 33 had inaccuracies in their MDS documentation regarding their pneumococcal vaccination status. Resident 15's MDS indicated the vaccine was offered and declined, but there was no documented evidence of education or consent. Residents 18, 31, and 33 had their MDSs marked as up to date despite incomplete or unclear vaccination records. Interviews with the Infection Preventionist (IP), Director of Nursing (DON), and MDS Coordinator confirmed these findings, indicating a lack of adherence to updated CDC guidelines and proper documentation practices.
