Summary

The facility failed to provide necessary care and services to ensure the residents' abilities in activities of daily living (ADLs) for two sampled residents. Resident 15, who has severe cognitive impairment and multiple diagnoses including bilateral primary osteoarthritis of the knee and Alzheimer's disease, was not provided with a communication device in their primary language and was observed in an uncomfortable and improper bed position. Despite the care plan indicating the need for alternative communication tools and proper positioning, these interventions were not implemented. The Director of Nursing acknowledged the lack of frequent visual checks to ensure proper positioning and the absence of a communication board at the bedside. Observations and interviews with staff and residents highlighted the lack of adherence to the care plans and facility policies, resulting in inadequate support for the residents' ADLs and communication needs.
