Summary

The facility failed to provide an environment free of accidents and hazards for three residents and two smoking areas. Resident 13, who had a history of unsafe smoking behaviors, was observed smoking unsupervised in the parking lot, with a lit cigarette propped between their lips and ashes falling on their lap. Despite previous incidents of smoking in their bathroom and being re-educated on the smoking policy, Resident 13 continued to smoke unsafely.
