Resident #9, who has multiple sclerosis and a left-hand contracture, did not have a comprehensive care plan addressing the management of the contracture. Despite the contracture being known since at least October 2023, there were no interventions in place to prevent potential complications. Similarly, Resident #65, who has severe cognitive impairment, did not have individualized care plans for the use of psychotropic medications or activities to accommodate their abilities. Lastly, Resident #71, who experienced significant weight loss, did not have a nutritional care plan in place, even though the facility's policy required one. The Registered Dietitian and MDS nurse confirmed the absence of a nutritional care plan, despite the resident's notable weight loss.
