Summary

The facility failed to ensure the Quality Assessment & Assurance (QAA) / Quality Assurance and Performance Improvement (QAPI) committee conducted performance improvement activities to sustain prior improvement measures for self-administration of medications. The QAPI plan indicated that the team would identify, collect, and analyze data from different departments, conduct root cause analysis, and develop a Performance Improvement Plan (PIP) with goals, actions, responsible parties, target dates, and status/outcome.
