Observations confirmed that his colostomy bag was extremely extended with gas. The care plan for Resident #11 failed to document a care focus for the maintenance of his newly placed colostomy. Staff interviews indicated that colostomy care was not being provided as needed, and the resident's concerns about the bag leaking were documented in progress notes. The CNC acknowledged that the facility had not provided staff with education on colostomy care, contributing to the deficiencies observed.
